WISECARD joining instructions

How do I become a cardholder or renew my card?

First check the eligible benefits section and if you are eligible to apply for a WISECARD you will need to provide for all applicants aged 8 years and over and children in receipt of Disability Living Allowance, the following:

  • One recent passport sized (2.5 x 3.5cm) photograph.
  • The correct proof of eligibility for each applicant (When applying by post, photocopies only).
  • Your completed application form.

Please note:

Children under 8 years of age will not be issued with their own WISECARD but their names and dates of birth should be included on the application form.

  • If you are renewing your WISECARD please bring your old card.
  • Every applicant will need to sign and collect their card personally (except on postal applications).
  • To obtain a family WISECARD the named benefit claimant must be present.
  • Postal applications only - cards will be available for collection from a WISECARD issuing centre 7 days from receipt of the completed application form. Please state which venue you wish to collect your WISECARD from.

Eligible Benefits

You will need to provide a payment book or letter of notification.

The following benefits entitle dependents of the main applicant to join the WISECARD scheme:

  • Pension Credit
  • Housing or Council Tax Benefits
  • Income Support
  • Working Tax Credit
  • Job Seekers' Allowance (Income Based). Ensure letter states JSA income

The following benefits only entitle the applicant to join the WISECARD scheme.

  • Job Seekers' Allowance (Contribution Based). Ensure letter states JSA contributions
  • Earnings Top Up
  • NHS HC2 Certificate
  • Attendance Allowance*
  • Disability Living Allowance*
  • Incapacity Benefit (Higher or Long Term Rate)*
  • Industry Injuries Benefit*
  • Invalid Care Allowance
  • Severe Disablement Allowance*
  • War Disablement Pension*

Wiltshire County Council Social Services

If you are registered under the Chronically Sick and Disabled Persons Act, you will need to show your letter of registration.*

* If you are in receipt of a disability benefit or registered with Social Services, please ask about helper entitlement when applying for a WISECARD.

Government Training Scheme

You will need to provide a letter from your employer stating that you are on a Youth Training Scheme and the duration of scheme.

Family Cardholders

Dependant Spouse/Partner

You will need to provide proof of address e.g. driving licence.

Dependent Child Under 16

You will need to provide a Birth Certificate or Passport.
