When submitting your application form, you need to provide the following information.
Proof of Child Benefit (if applicable) i.e. Child Benefit Book, latest full Letter from Child Benefit Agency, showing award amount and effective date or latest Bank/Building Society Statement, clearly showing.
If your child(ren) attend child care, then please supply letter from child minder, on their headed paper with their registration number, stating amount charge, hours of attendance, name of child(ren) and for what period.
Unless they are not related to you, please supply proof of income for each person listed here. i.e., latest wage slip, benefit payment book, letter of confirmation of attending college etc.
Please inform us whether you are waiting to hear or receiving Income Support or income based Job Seekers Allowance.
If
you or your partner are unable to supply wage slips, or if the wage slips you have are handwritten,
you can use the
Housing benefit and council tax benefit - Certificate of earnings form 26kb (to be completed by your
or your partner's employer).
Please supply proof of all benefits, i.e. State Retirement Pension Book, Industrial Injuries Benefit Book, War Widow's Pension Book, Income Support Book or latest FULL letter from Department of Works & Pensions (formally Department of Social Security - DSS) for each benefit you receive or complete the top part of the enclosed DWP Confirmation Form and have it endorsed by the Department of Works & Pensions.
If you receive a private pension(s), please supply latest pension advice slip.
If you are receiving Working and/or Child Tax Credit, please supply latest full Tax Credit Letter.
If you are receiving maintenance, please supply latest full letter from Child Support Agency showing award amount and effective date or a letter from ex-partner, stating amount, frequency and reason.
Please tell us if you have nay other monies coming in, such as from lodgers or sub-tenants.
Please supply all capital, savings and investments, for instance current/savings accounts with banks/builidng societies or post offices, premium bonds and shares.
If you have a 2nd property, please provide proof of current market value. If you are renting it out, please provide proof of current tenancy agreement, showing rent charged. Proof of outstanding mortgage and monthly mortgage repayment.
If you not are renting from a Private Landlord or Housing Association go to part 14.
Please fill in parts 11 to 13 and supply your latest up to date FULL tenancy agreement or have your landlord complete the available landlord form.
Please supply information about the property you live in.
Who do you wish your benefit to be paid to yourself or your landlord (if you have one).
